Must admit I can’t wait to see the effect on the NHS of that 350M that’s going to start flowing in a couple of weeks. That’s definitely going to help mitigate the loss of freedom of movement.

The coroner said "The safety net that should surround vulnerable people like Errol in our society had holes within it”.
He was failed by a huge range of services that should have helped him like social services, GP, landlord, etc etc so pinning it on IDS is, at best, tenuous.

Not the least bit surprised as I've experience of them along these lines and they are a f*****g joke. Did you miss the bit where he wasn't asked to fill in an ESA questionnaire as I read originally? After all this was a person who was on benefits for this long term.

It was incompetence on their part, along with other organisations involved. The DWP are still a joke though with absolutely no flexibility and struggle with conditions that aren't standard (from my personal experience). I would dread having to go to the DWP with a mental health issue with their lack of understanding.

Sadly (irrespective of politic`s) in that period of evolution problems will be found, it is how they are dealt with that matters more.
The last part ,is that any system is operated by humans, and people will (but should not) fall through the gaps.

How long does that change go on where people lose their lives as a result as this isn't unique as that report and many others have pointed out?

This change process has been going on for over 10 years for instance.

This is somebody who clearly had significant mental health issues and mental health issues don't fit neatly into tick boxes. The comments/explanation boxes in the questionnaire might as well not be included for how much notice/weight is put on them in the process as I found out, hence my comment about unusual conditions not fitting the system. The system is a joke as it stands unless the intention of the system is to create a "hostile environment" for potential recipients.

Not an excuse but the law of averages says something will go wrong with that volume of transactions.
PIP re assessment department are one of the few government departments that are really not 'user friendly' - wheras most government run departments have been dealing with 'claims' etc ( inland revenue for example with tax returns etc) on line, the PIP assessors send out reassessment forms by 2nd class snail mail - Which can and often does take up to 2 weeks to arrive- requesting/ demanding that the claim form is returned within 4 weeks of the date it was sent, with GP and specialist care reports (hard copy) included, which is a near impossibility to comply with, as 1) GPs tend not to issue reorts without first having had a face to face consultation with the claimant, and getting an appointment to see a gp in under a fortnight is not possible, ditto obtaining reports from other healthcare professional is equally non possible in a short time frame . 2) the PIP claim form comes with a 2nd class prepaid envelope, which , once relevant documents/ repoerts are eventually collated and included, it will take (again) up to 2 weeks from posting to arriving with the PIP assessors.

therefore the PIP reassessment claim form, and accompanying documents, arrive 'late' and then theyre in a queue to be assessed and processed, in the interim the cut off date given has long expired, which sees the PIP claimants PIP payments stopped on the unrealistic cutoff date. Leading to stress and pressure on vulnerable people due to their benifits being stopped forthwith, even with successful re assessment claims, it takes a month or more for PIP assessors to process and make a decision.

IF PIP re assessors were to offer an online option or even to use first class post, time limits would be much more user friendly and also much less stressful for those being reassessed. Cynic that I am, i do wonder if there is some kind of strange reasoning as to why PIP re assessesments arent tailored to be as helpful as they conceivably could, maybe should , be ?
